Diabetes Resources for Health Care Professionals

The Improving Diabetes and Obesity (iDo) coalition created several fact sheets in English and Spanish for health care providers and the community.

These fact sheets were adapted from New Mexico Health Care Takes on Diabetes fact sheets, to help those with diabetes:

  • Better manage their care
  • Know their numbers
  • Learn about important community resources including tobacco cessation
  • Learn the importance of requesting annual exams
  • Partner with their health care providers
